Be sure to answer all questions completely. Applications and supplemental materials are to be returned to Guidance by 1/17/14.
Students must return a copy of the application and all supporting documents for each scholarship they
want to be considered.

Please read carefully. If materials are missing from your packet it will NOT be considered. You must submit one completed set of
materials for each scholarship that you wish to be considered. It is your responsibility to make these copies. Your set of materials

INSTRUCTION 2: (Community Scholarship Application - Required)

A completed copy of the Community Scholarship Application is required for each scholarship that you wish to be
considered.
INSTRUCTION 3: (Resume - Required)
Use any format. Guidance does NOT keep records of previous awards, honors, etc. Resume MUST include the following:
 Scholastic honors received or earned
 Other honors received or earned (community, church, scouts, athletics, etc.)
 Clubs and organizations you have belonged to, the number of years you belonged, and positions of leadership, if any
(school, community, church, etc.)
 Part-time work and/or community volunteer experience you have done during the past three years (include employer,
position, length of employment, and hours worked or volunteered)
INSTRUCTION 4: (One Letter of Recommendation - Required)

Please provide one letter of recommendation from a teacher, administrator or non-relative unless otherwise specified
within the criteria listing. Guidance counselors will NOT provide recommendations for scholarships.
INSTRUCTION 5: (Optional Information)

It is permissible to include a letter of explanation of any special circumstances, such as academic obstacles overcome or
financial or family changes which you would like the committee to know.
INSTRUCTION 6: (Vocational Essay - Required)
Note: All students must complete the following essay in addition to all other required instructions.
 Please attach a typewritten, double-spaced essay (300-500 words) stating why you have chosen to pursue your proposed
occupation or vocation, what your present adult goals and objectives are, what you feel are your personal assets and
strengths, and why you feel you are deserving of a scholarship to continue your education.

INSTRUCTION 8: (Unofficial Transcript - Required)

Upon completion of Instructions 1 – 7, students are asked to bring their scholarship packets to Guidance where they will
receive the appropriate number of Unofficial Transcripts and Testing Pages to attach to their Application packets. Students
will IMMEDIATELY submit these completed packets to Guidance. NOTE: To avoid a long last minute wait, make every
attempt to complete Instructions 1-7 promptly.

Please read the eligibility requirements for each scholarship. You may apply for as many community scholarships as you wish
provided you meet the qualifications.
Students must provide a copy of all materials for each scholarship for which they want to be considered (i.e., if you are
applying for 3 scholarships you must submit 3 sets of all required materials). Please keep an additional master copy of your
application materials for your records! Completed applications and all supporting documents must be returned to the Guidance
Office by January 17, 2014.
District Wide Scholarships:
One and/or more scholarships will be awarded to students in any of Westerville’s three high schools.

ALLIANCE DATA: Two, $1000 Scholarships available to students in Westerville City Schools. Students must have
between a 3.0 and 4.0 weighted GPA.

BLENDON LODGE #339 F.&A.M. SCHOLARSHIP: Three, $500 scholarships will be awarded to seniors in any
of the three Westerville high schools who possess at least a 3.0 cumulative grade point average. This scholarship takes
into consideration academic achievement, letters of recommendation, extra-curricular activities, community service
and financial need. The following information is required and should be included in step 5 of the Community
Scholarship Application: 1) Gross Family income; 2) Any known or expected college financial assistance. Winner
will be expected to provide an invoice from the college for payment.

COLUMBUS FOUNDATION/THE JEFFREY COPELAND ANNUAL SCHOLARSHIP AWARD FOR
ENGINEERING STUDIES. Candidate must be a graduating senior of a Westerville City Schools High School.
Candidate must plan to pursue a degree in engineering. Candidate must have at least a 3.0 cum GPA by the end of
their first semester of their senior year. Candidate must plan to attend a 4 year college or university with preference
given to those attending a “Big 10” college or university. In addition, a copy of the SAR is required, completion of
the FAFSA form is required and the applicant must be willing to provide a copy of the Student Aid Report to the
interview committee.

DAVID FINKELSTEIN MEMORIAL SCHOLARSHIP: This 2 year scholarship of $8,000-$10,000 is divided among
4 award recipients. Applicant must write a brief explanation of how the death of a family member or friend affected
his/her choice of colleges, degree pursuits, and/or life goals. Four winners receive $500 each and an interview. An
additional $2,000-$3,000 will be divided among the candidates after the interviews. In addition, after completing year
1 of college, the 4 winners will interview for a share of an additional $4,000-$5,000.

GENOA TOWNSHIP BUSINESS ASSOCIATION: One, $1000 one-time Scholarship is available. A graduating
senior who resides in Genoa Township and is continuing his/her education at an accredited post-secondary institution
is eligible to apply. Applicant must complete the Genoa Township Business Association Scholarship Application
located at: www.genoatba.com and a supplemental one page, double spaced 12.5 font essay describing how postsecondary opportunities and receiving this business association scholarship will help you give back to Genoa
Township. Deadline for submission is March 8, 2014. Students submitting AFTER January 17, 2014, must send
directly to: Genoa Twp. Business Association, c/o GTBA Scholarship; 5111 S. Old 3C Hwy; Westerville, OH 43082.
NOTE: Letter of Recommendation must be from a non-relative.
_____ THE LORI SHERMAN MEMORIAL SCHOLARSHIP: This $250 Scholarship was created in memory of Lori
Sherman, better known to McVay students as, “Our Lunch Lady”. Lori’s positive and generous nature allowed her to
use the ordinary moments of the school day to make extraordinary differences to all at McVay. This $250 Scholarship
will be awarded to a graduating senior of Westerville City Schools who attended McVay Elementary for a minimum
of 3 years. This academically deserving student must have a military commitment or have been accepted to an
accredited college, university or vocational school.

THE MIKE CHUDIK MEMORIAL BUSINESS SCHOLARSHIP: Two, $500 scholarships will be awarded: 1) One
male winner and one female winner; 2) Student must be pursuing a business or entrepreneurship major at a 2-year or

OHIO PTA MEMORIAL SCHOLARSHIP: Three, $500 scholarships awarded in Ohio District #10: One Academic
Scholarship available to students pursuing a Bachelor’s Degree; one Technical Scholarship available to students
pursuing a 2 or 3 year or Associate’s Degree; and one Special Education Scholarship. Criteria based on student’s
scholastic achievement, school involvement, leadership qualifications, and special family circumstances. Ohio PTA
Applications for each of these Scholarships are available in the download section under Scholarships at

SUCCEED SCHOLARSHIP: Two, $750 Scholarships are available. Stolly Insurance Group’s Succeed Committee
has awarded two scholarships for $750 each for the past nine years and is looking forward to announcing the next
winners on April 25, 2014. Eligible students can locate the supplemental scholarship application at www.stolly.com.
Students will find a brief explanation of what the Succeed Committee does, the eligibility guidelines, as well as the
application. The application MUST be mailed or E-mailed, and has an entry deadline of February 10, 2014 –
address and E-mail information are on the supplemental scholarship application.

TUTORING CLUBS OF CENTRAL OHIO: One to three scholarships ranging from $350 to $1,000 each will be
awarded to graduating seniors from Westerville North, South and Central high schools. Applicants must have a
minimum 3.5 cum GPA (4.0 scale), as well as a minimum ACT composite score of 24 or an SAT score of 1700.
Extra-curricular activities, academic achievements, and community service will also be considered.

WESTERVILLE ALUMNI ASSOCIATION / FAITH PRUDEN MEMORIAL SCHOLARSHIP: This $1500
Scholarship will be awarded to a graduating senior who will be attending a traditional or non-traditional college
studying cosmetology, the arts, culinary studies or fire services.

WESTERVILLE CHAMBER FOUNDATION JIM NEAR OTTERBEIN SCHOLARSHIP: This is a $1500 award
for a student who will be attending Otterbein College in the upcoming year. The focus is on students who have
demonstrated leadership and/or outstanding participation in volunteer, charitable and community service
organizations. All applicants must supply THREE copies of the application and supporting materials. Finalists must
attend an interview with Chamber Foundation members.

WESTERVILLE CHAMBER FOUNDATION JIM NEAR TECHNICAL/TRADE SCHOOL SCHOLARSHIP:
This is a $1500 award for a student who will be attending a technical or trade school. The focus is on students who
have demonstrated leadership and/or outstanding participation in volunteer, charitable and community service
organizations. All applicants must supply THREE copies of the application and supporting materials. Finalists must
attend an interview with Chamber Foundation members.

THE WESTERVILLE FIRE FIGHTERS’ MEMORIAL SCHOLARSHIP: A $1,000 scholarship will be awarded to
an individual pursuing a career in Fire/Emergency Medical Services (EMS). Applications will be evaluated on the
following criteria: academic achievement, community and school involvement, financial need, relevance of field of
study, and an explanation of how applicant plans to make a difference with his/her career plans. To download the

WESTERVILLE ROTARY CLUB OTTERBEIN SCHOLARSHIP: This is a cumulative $8000 scholarship offered
to a qualified Westerville area high school senior who will be attending Otterbein University as a full time student.
This 4 year scholarship will be awarded over a four year period at $2000 per year. 1) Applicants must be high school
seniors who reside in the Westerville school district and expect to be graduating during the year of the award. 2)
Applicants must be eligible for acceptance by Otterbein University. 3) Children and grandchildren of Westerville
Rotarians are not eligible for this award. 4) Consideration will be given to personal reputation, academic
accomplishments, and extracurricular activities and to a lesser extent, financial need. The Rotary Scholarship
Committee will study all applications that are timely submitted and may, at their discretion, invite applicants for
personal interviews. Committee decisions are final and not subject to appeal. All winners will be notified in writing.

attend Otterbein University and participate in music at the university (orchestra, band, etc.). Applicants should have

A BETTER TOMORROW: One, $500 Scholarships will be awarded to students based on the following criteria: All
applications must be submitted in a format which showcases the student’s creativity and imagination (Example:
Audio, video, picture, poem, song). It must relate to the theme “A Better Tomorrow”. A written explanation may be
included to help explain the relevance but it is not necessary. All written application materials must be submitted to

teacher/staff member and a one page essay explaining why applicant’s personal qualities make them the best recipient

LEROY A. GILKEY MEMORIAL SCHOLARSHIP: One, $1500 award. Must have a cumulative GPA of 2.5 or
higher; plan to pursue higher education in an area related to World Language/Studies or participate in a related travel
experience. In addition to the Community Scholarship application, applicant must submit a one page essay explaining
why the applicant’s personal qualities make them the best recipient of this scholarship (essay may include financial

GREG FARNHAM MEMORIAL SCHOLARSHIP: Two, $6000 awards to a male & a female applicant. The
applicants must be scholar athletes, earned a minimum Cum GPA 3.0, earned a varsity letter in a sport and plans to
attend an Ohio college or university. Submit three letters of recommendation; one from a teacher, WNHS varsity
coach, and a peer. Submit a personal letter to the selection committee explaining interests, values, and career goals.
Recommendation forms are located in the guidance office.

SANDY LARGENT MEMORIAL SCHOLARSHIP: One, $4000 ($1,000 per academic year, per person). The
applicant must demonstrate outstanding achievement in and out of the classroom and plan to continue that effort while
attending The Ohio State University. Must have a 3.3 cum GPA or higher, be actively involved in extracurricular or
community activities outside the classroom, has been accepted to OSU and has committed to attend the university in
pursuit of a four year degree program. Submit a one page essay (standard format) stating how your family and/or
friends have supported you throughout your schooling, how you plan to carry on that philosophy, and why that should
be a key to your success as a contributing member of society’s betterment.

school including but not limited to manual trades, health care and computer technology. Factors for consideration will
include but are not limited to: academic achievement, extra-curricular activities and leadership, community and
volunteer service, financial need, as well as any other personal and family considerations.

WESTERVILLE PARENT COUNCIL PTA: A minimum of three, $500 scholarships awarded: one or more per high
school depending on available funds. All students graduating from a Westerville City Schools high school are eligible
and encouraged to apply. Factors for consideration will include but are not limited to: academic achievement, extracurricular activities and leadership, community and volunteer service, financial need, as well as any other personal and
family considerations.

WESTERVILLE PARENT COUNCIL SCHOLARSHIP FOR EXCEPTIONAL STUDENTS: One, $500 Scholarship
is available to a graduating student in Westerville City Schools. All students who are enrolled in the Special
Education Program are encouraged to apply. Applicants for this Scholarship must have a current IEP OR 504 on file.
Students must be pursuing a post-secondary education. This scholarship is based on academic achievement,
community service/volunteerism, extra-curricular activities/leadership, letter of recommendation and financial needs,
as well as any other personal and family considerations. The WPC Scholarship for Exceptional Students
Scholarship Application MUST BE COMPLETED in lieu of the Community Scholarship Application. It is

ALCOTT PTA: Alcott Elementary is offering two (2) $500 Scholarships to two student at any Westerville City
Schools high school who attended Alcott Elementary for at least three years. The following criteria must be met to be
eligible: They must have attended Alcott Elementary for at least three years; they will be going to college; they must
complete the Community Scholarship Application Form.

CHERRINGTON PTA: One, $500 scholarship. Must have attended Cherrington Elementary for at least two
complete school years. Must have a current GPA of 2.7 or better. Please address your financial need in a 50-100
word supplement to your essay.

EMERSON PTA: One, $500 Scholarship awarded. Student must have attended Emerson World Languages and
Cultures Elementary Magnet School for at least 1 full academic year and be accepted at a 2 or 4 year college or
accredited university.

FOUSE ELEMENTARY: One, $500 scholarship available. Student must have attended Fouse for at least 3 years.
Please include an additional essay stating how Fouse Elementary played a part or influenced who you are today.

HANBY PTA: One, $500 Scholarship is available. 1) Student must have attended Hanby Arts Magnet in either the
Magnet or A&T program. Applicant must have attended Hanby Arts Magnet for a minimum of two full time
academic years. 2) Student must be accepted in an accredited post secondary program or institution by the time of
scholarship disbursement.

HANBY PTA: One, $500 Scholarship available. Student must have attended Central College Magnet School for a
minimum of three years.

HANBY PTA: One, $500 Scholarship is available. 1) Student must have attended Longfellow Magnet School for
at least one year. Special consideration is given to those who plan to study math, science and /or education. We also
request an essay about your time at Longfellow and how it has impacted your life and your career choice.

MARK TWAIN PTA: Two, $500 Scholarships are available. Student must have attended Mark Twain for a
minimum of one year. An emphasis is placed on academic achievement and community service. Please include a
short description of your experience at Mark Twain and any community service projects you have been involved with
during your high school years.

MCVAY PTA: Two, $500 scholarships are available. Student must be graduating from Westerville City Schools’
high school and have attended McVay Elementary School for a minimum of 3 years. Student must be accepted at an
accredited college, university or vocational school or have made a military service commitment.

ROBERT FROST PTA: Two, $750 scholarships are offered to a graduating senior who resides in the Westerville City
School District. Must have been accepted at a university or 2 year post high school and must have attended Robert
Frost Elementary for a minimum of 1 complete academic year in grades 1-5. A separate application and essay are
required and this information will be available on-line to download in or around mid-December from the Robert Frost

WHITTIER PTA: One, $500 Scholarship is available. Student must have attended Whittier Elementary for 5
academic years. In addition, student is required to submit two letters of recommendation from teachers or leaders in
the community.

WILDER PTA: Two, $750 Scholarships are available. Student must have attended Wilder Elementary for at least 4
school years and must submit a cover sheet that includes their years at Wilder, the names of their teachers and their
plans for after graduation. Students also need to submit an essay on "How Wilder Changed My Life". Grades are
important, but so is involvement in extra-curricular activities and service to others.
MIDDLE SCHOOL SCHOLARSHIPS:

BLENDON PTO: Two (2), $500 scholarships will be awarded. Student must have attended Blendon Middle School
for at least one academic year. Student must have at least a 3.0 cum GPA.

GENOA PTSA: Three (3), $500 academic scholarships will be awarded to graduating seniors district wide. The
applicant must have attended Genoa Middle School for at least one full academic year. Please include a brief
statement of your favorite memory at Genoa.

HERITAGE PTSA: One, $500 Scholarship will be awarded to a graduating senior from Westerville North, South or
Central High Schools. Applicant must have attended Heritage MS for a minimum of one academic year.

WALNUT SPRINGS PTSA: Two, $250 scholarships are available. Student must have attended Walnut Springs
Middle School for at least one complete academic year. An emphasis is placed on community service. Please include

HIGH SCHOOL SCHOLARSHIPS:

WESTERVILLE NORTH ATHLETIC BOOSTERS: $500 Scholarships are available (additional scholarships may be
available based on funding) for seniors who have a 3.2 cum GPA or higher and have lettered for at least two years
(lettering in several sports a plus). Class rank, community service projects and a letter explaining the reason you
deserve a Scholarship is required.

WESTERVILLE NORTH ACADEMIC BOOSTERS: At least two, $500 Honor Scholarships awarded to students
with 3.7 or above cum GPA. At least two, $250 Merit Scholarships awarded to students with a 3.3 - 3.69 cum GPA.
Each of these Scholarships require an additional essay which is limited to one typewritten page, double spaced,
discussing the student's most memorable moment at North.

WESTERVILLE NORTH MUSIC BOOSTERS: Two Scholarships are available. One, $500 Choir/Band/Orchestra
Scholarship is available to a student who has participated for at least one year in one of the music programs at WNHS
including choir, band or orchestra. Also, must be pursuing a secondary degree in Music. An additional $250
Scholarship is available to a student who has participated for at least one year in one of the music programs at WNHS.
Any choir, band (including marching band) or orchestra applies.
_____ WESTERVILLE NORTH THEATRE BOOSTERS: One, possibly two, $500 scholarships are available. Student must
have attended Westerville North for a minimum of one academic year, must have been inducted into the International
Thespian Society, and must have participated in Westerville North’s Theatre Program. Student must submit a short
(1-2 page) essay explaining how theatre has impacted their life and why they are deserving of the Westerville North
Theatre Boosters Scholarship. Applications and essay are not due until March 17. NOTE: Number of scholarships is
based on money earned from Senior One Act Plays Fundraiser. Students must also complete WNHS Theatre Boosters
